Project goals
To develop effective policies, strategies and action plans in the priority issues of the reforms and the project; to enchance collaboration between health and social welfare sectors; to improve strategic and financial management of health and social services; to improve design and implementation of legislation; to improve the system of information gathering , analysis and utilisation; to develop human resource policies enabling appropriate staffing of services; to increase awareness of the reform process among citizens and professionals; tp develop models of primary health care and community-based social welfare. |

Results
The overall objective of the Project was to enhance the well-being of the people of the Republic of Karelia through support to health and social welfare service reforms, which are expected to lead to the implementation of efficient, effective, flexible and high-quality consumer-oriented services.
The project established five community integrated health centres focusing on the restructuring of the health care system more towards community-based approaches. The strongest components of this project were the legal reform in the health sector, the developing of the integrated approach between the social and health sectors at the republican and community levels and establishment of the republican network with the newly developed software. |

To complete the process of establishing alternative working models of community integrated health centres (CIHCs)To complete the process of establishing a network of model social care services supporting the most vulnerable sections of the populationTo demonstrate through practical example how district community health and social care services can be better integrated at planning, management and service delivery levelsTo provide the essential pre-conditions under which the achievement of all previously described objectives can be successfully sustained by the Karelian authorities after the end of the current project
