Objectives:  The project aims at setting up a joint teaching programme lead by a project group in which the Swedish members will be nurses representing HIV care and hospital hygiene and similar competency from Russia. The project group will recruit suitable trainers, design a teaching programme and oversee the implementation. It is aimed that the programme will be made available to head-nurses in above mentioned groups of nurses obligating them to spread their knowledge in their home district. Russia has the most rapidly raising HIV incidence in the world. At the end of 2002, there is estimated to be about 1 miljon HIV-positive Russians, most of them infected through intravenous drug use (IDU). There is no sign of any slowing-down. Most Russian health care workers (HCWs) are still unaccustomed to dealing with HIV-positive patients. The situation is very similar to the situation existing in Sweden 15 years ago. The problems include concern about own personal safety (often expressed also by spouses of the HCWs), lack of care routines, and inexperience in talking to patients about protecting others.

In Sweden, there exists a large group of nurses with decade-long experience in care for HIV patients. Many members in this group have expressed their willingness to participate in a programme to share this experience with their Russian counterparts. Comments The present rapid spread of HIV infection in Russia is leaving much of the health care staff behind due to experience and skill. In many ways, the situation is similar to Sweden in the mid 80s. A wish has been expressed by representatives from the Russian nurses collective that Swedish nurses could share their experience and knowledge on a larger scale. This knowledge would include care routine, knowledge of counselling and hospital hygiene, but also focus on preventive aspects of carefully informing already HIV- positive patients about measures not to infect others.

Objectives:  A particulary exposed group in Russian society is young people. HIV/AIDS and other sexually transmitted infections (STI) and also abortions are prevalent to a large extent. To reach young people via youth friendly services (youth clinics) and sex education at schools are therfore important. The overall objectives are to prevent sexually transmitted infections including HIV/AIDS and unwanted pregnancies among young people in St Petersburg. Partial objectives are: develop four youth clinics and use the knowledge and experience of the already established youth clinics to supervise staff; create an academic postgraduate course for youth clinic staff, establish a reliable distribution of contraceptives and condoms so young people have access to cheap and safe preservatives, create a basic course for medical and school staff about sexual health, create an academic method course for school staff, participate in an exchange of ideas about safe sex in existing programme at summer camps, influence massmedia about the importance of sexual education, co-operate with NGOs about the importance of sex education for young people. Objectives:  Antibiotic resistance in clinically important bacteria is an emerging and worldwide problem. It severely affects the usefulness of antibiotics in treatment of bacterial infections. One important step in the control of this problem is to apply standardized methodologies and interpretation criteria and to introduce regular surveillance programs of antibiotic resistant bacteria. Surveillance programs based on routine susceptibility data should involve clinical bacteriology laboratories serving defined areas of the country. To initiate a surveillance program the following parts should be attended to: (i) collecting relevant clinical isolates for susceptibility testing, (ii) registering quantitative data, (iii) analysing computerized data, and (iv) modifying and improving methodology and interpretative criteria, if necessary. Regular external quality assurance of methods should be implemented. As a complement to the surveillance system it is necessary to introduce modern DNA-based methods for detection of resistance. These methods could be introduced during an applied training program and need funding in terms of equipment. At the national level, these advanced methods should preferably be centralized to a reference/referral laboratory.
