Project goals

To develop GP-based primary health care services in four pilot regions; to develop a management culture in Russian hospitals; to develop a demonstration project on maternal and child health; to develop economic evaluation techniques and operational research skills in the pilot regions; to spread information on the pilot interventions nationally.

Results

Fifteen GP offices were established and equipped with Project support, proved to be effective still four years after Project completion. Moreover, Samara and St. Petersburg are today at the forefront of GP based primary health care. This is certainly not the outcome of this one project only, but the results of various activities in this area over an extended period.


The project demonstrated how circumspect research should accompany the structural reform. The outcome of this work submitted valuable information about advantages and disadvantages of the model and, thus of issues that deserve particular attention while implementing the GP-based service model. It is concluded from the outcome of this Project, that structural and operational changes in the health care system need to be assisted by critical monitoring that helps adapting new services to the real requirements.


It is recommended, from the conclusions derived from this Project, to use for replication practical experience gathered in establishing GP offices in the pilot regions of this Project. This, however, should be accompanied by i) medium-term strategic planning for wider coverage of the population with GP services over a defined period of time, and ii) new management structures and functions of both the out-patient and the hospital sectors.
